You are being invited to participate in a research-use cohort hosted on
the Peptide MD platform. Please read each numbered point below; ask the Host
for clarification on any item before signing.

1. NATURE OF THE STUDY
This is a research-use protocol, not a clinical treatment. The protocol
classification is shown on the cohort page. The peptide(s), dose schedule,
and monitoring endpoints are pre-registered and may not be altered after
the OEA-V1 commit.

2. RISKS
The peptide(s) listed for this cohort may have known and unknown risks,
including but not limited to: injection-site reactions, immunological
response, drug interactions, and regulatory exposure based on your
jurisdiction.

3. PARTICIPATION IS VOLUNTARY
You may withdraw at any time before the reveal window opens by submitting
a tombstone leaf. The original leaf hash cannot be removed retroactively;
your participation is recorded as anonymized.

4. PRIVACY
Your identity is never published. The Cohort Merkle root binds a SHA-256
of your canonical bundle, including a salt that stays on your device. The
Host receives only the leaf hash.

5. DATA USE
Wearable telemetry and PRO uploads are used solely for the pre-registered
primary endpoint. Aggregate data may be cited in revealed dossiers; your
individual data is never released without your separate consent.
